Community Assessment Hub focussed for people experiencing COVID-19 symptoms is now up and running in Grampian.
The newly established Community Assessment Hub will provide people with a straight forward and dedicated route to clinical advice and support when experiencing symptoms associated with COVID-19. The Hub can only be accessed through NHS24 by calling 111 day or night.
The introduction of the Grampian Community Assessment Hub will free up Primary Care teams to continue to treat and care for non COVID-19 related health and continue to provide support and treatment when appropriate for people who may be unwell with COVID-19 at home..
People with symptoms related to COVID-19 should:
- Phone NHS24 on 111 if their symptoms worsen or do not improve after seven days.
- Following an assessment, callers may be given advice to help them continue self-isolating at home or their call will be transferred to the hub where a virtual clinical assessment with access to specialist Doctors will be carried out, this will approach will make use of technology as a means of a more in depth assessment. At this point you may be admitted to hospital or referred back to the GP practice and local health and social care teams for care,
- Should there be a clinical need to be seen by a healthcare professional to assist decision making callers may be given an appointment to attend a local assessment centre within a hub.
The Hub assessment centre will be open on an appointment only basis and will not be open to ‘walk-ins’ Anyone concerned about their symptoms should call NHS24 and should not present to any hospital or GP surgery.
